Please call, text or email me first, before booking, if you plan to use insurance otherwise you will need to self pay. I don't bill health insurance​, however I do provide all the necessary documentation for reimbursement for health insurance. I do bill most auto insurance directly. All insurance clients must receive treatment for a diagnosed medical condition.  I base my treatment on that diagnosis. You will need to get a doctor's prescription before insurance will cover you on most plans.

Forms you should reference when billing your insurance:

• auto insurance intake form
• prescription form (only if you don't have a prescription from a doctor already)
• insurance pre-approval form (for health insurance reimbursement)
